What is Electrophysiology Test?
Electrophysiology studies (EPS) are diagnostic examinations that help doctors figure out what causes irregular heart rhythms (arrhythmias). Electrophysiologic testing involves inserting catheters into the heart to determine the source of irregular heartbeats.
The contractions of the various components of the heart are coordinated by natural electrical impulses. This aids in the proper circulation of blood. The heartbeat, also known as heart rhythm, is the result of this movement of the heart.
During the EP test, heart rhythm specialists or electrophysiologists map the propagation of the heart’s electrical impulses during each beat. This is done to figure out what’s causing an irregular heartbeat.
These findings may assist you and your doctor in determining if you require medication, a pacemaker, an implanted cardioverter defibrillator (ICD), cardiac ablation, or surgery.
These tests are performed while you are sedated in a special room called an electrophysiology (EP) lab or a catheterization (Cath) lab.
When is Electrophysiology Test required?
When someone’s heart doesn’t beat normally, doctors use EPS to find out why.
Doctors use EPS to see:
- Where does an arrhythmia originate?
- How effective certain medications are at treating your arrhythmia.
- If the problem can be treated by eliminating the part of your heart that is causing the aberrant electrical signal, medically known as Catheter ablation.
- If you may benefit from a pacemaker or an implanted cardioverter defibrillator (ICD)
How is Electrophysiology Test done?
Step 1
You may be given drugs intravenously, or through your arm, to sedate or make you sleepy, depending on the type of study you are involved in. These drugs can help you feel less anxious and relieve pain. A local anesthetic will be used to numb the area. A needle will be inserted through your skin and into a blood vessel by your doctor.
Step 2
A sheath, a little tube about the size of a straw, will be put into your artery or vein. Several specialised EP catheters will be softly guided into your blood vessel through the sheath and advanced to your heart by the doctor. There are two parts to the EP study:
- Recording the heart’s electrical signals to assess the electrical function
- Pacing the heart to bring on certain abnormal rhythms for observation under controlled conditions
Step 3
Your doctor will use the catheters to deliver small electric pulses to your heart, causing it to beat at different speeds. The unique catheters will pick up and record electrical signals produced by your heart. This is known as cardiac mapping, and it allows the clinician to pinpoint the location of arrhythmias.

Electrophysiologists deal with electricity, specifically the electrical impulses that control the heart’s rhythm and cause heartbeats, as the name implies. Arrhythmias are diagnosed and treated by an electrophysiologist.
Patients are generally referred to an electrophysiologist by a physician or a cardiologist when they are diagnosed with an arrhythmia. Arrhythmias can be Bradycardia—a heart that beats too slowly, or Tachycardia—a heart that beats too fast.
Typically, electrophysiologists defibrillate the heart (give it a regulated electrical shock) to reset the electrical system, and then implant a pacemaker or defibrillator to keep it running smoothly.
