What is Total Hip Replacement Surgery ?

Hip Replacement Surgery involves replacing the hip joint that has been damaged or worn away by arthritis or injury with artificial hip parts or implants called prosthesis. The prosthesis substitutes for the hip joint allowing smooth and frictionless movement. The goal of hip replacement is to relieve pain and increase the mobility and function of a damaged hip joint.

Hip Replacement is also advised as the golden standard for hip arthritis in patients older than 65 years and with patients with advanced hip destruction or altered anatomy of the hip.

The most common indication for undergoing hip replacement is usually arthritis.

  • Osteoarthritis is wearing of joints due to age occurring at or over the age of 50 years. The cartilage lining the hip bone is worn out, leading to exposed bones that rub against each other causing pain and mobility problems.
  • Inflammatory arthritis like rheumatoid arthritis and psoriatic arthritis are autoimmune diseases where the synovial membrane gets inflamed and thickened causing pain and stiffness
  • Post-traumatic arthritis which is caused due to an injury or fracture of the hip ultimately resulting in cartilage damage.
  • Childhood diseases of the hip cause serious hip problems and disfigurement of the hip in adulthood.
  • Avascular necrosis is caused by dislocation or fracture or any such injury that causes obstruction of the blood supply to the femoral head.
  • Bone tumors
Nonsurgical treatment options are:
  • Physiotherapy
  • Pain medications
  • Ayurveda Treatments
  • Glucocorticoid injections into the joint, anti-rheumatics and other medications that are beneficial

Surgical Options

Surgical treatment alternatives include:

  • Hip resurfacing: Advanced arthritis is treated conventionally with hip resurfacing surgery. Unlike the traditional hip replacement, the head of the femur is trimmed and capped with the metal covering. Also the damaged bone and cartilage is removed from the acetabulum and is replaced with the metal socket.
  • Hip Fusion: Hip fusion procedure eliminates all motions at the hip joint by fusing femur and pelvis together held in position by metal screws and plates.
  • Hip Osteotomy: This procedure realigns the hip joint bones and relieves early arthritis symptoms.
  • Resection Arthroplasty: The head of the femur bone is removed and in contrast with the other hip replacement surgery, the bone is not replaced but instead it is allowed to heal and develop its own fibrous scar tissue

When is Total Hip Replacement Surgery required ?

If you are suffering from severe pain, decreased range of motion or deformity at the hip joint and other medical and conservative treatments like physiotherapy have failed, you need a replacement of the hip joint.

If you have had hip injury, bone tumor or bone loss due to avascular necrosis (insufficient blood supply) or suffering from rheumatoid arthritis, your surgeon may advise you replacement of the hip joint.

How is Total Hip Replacement Surgery done?

Step 1

An incision is made on the lateral side of the hip. The upper round part of the thigh bone (head of the femur) is removed and replaced with an implant.

It is then stabilized into place by cementing or other methods. The part of the hip joint receiving the ball component called a socket or acetabulum is smoothened and prepared, and the implant is then placed to position and stabilized.

Step 3

The new ball and socket of the joint are then joined.

If for some reason you are not a candidate for hip replacement surgery, or do not wish to undergo hip replacement, there are effective non-conventional treatment options for you to get relief from your hip pain- Ayurveda treatment.

Ayurveda is an ancient Indian medical practice that dates back over 3,000 years. Internal cleansing is the first step in Ayurveda treatment, followed by a particular diet, herbal treatments, massage therapy, yoga, and meditation. There are various Ayurveda treatment options available to completely cure hip pain. Talk to your case manager and he/she shall be able to help you.

There is no right or ideal age to get the hip replacement done. If you cannot bear pain any more and the other options are not working, you can get your hip replaced irrespective of your age. The average age of hip joint replacement patients is around 60 years old; however, patients of all ages have received hip implants.

THR used to be an option mainly for adults after 60. But improvements in technology has made implants or artificial joints that last for life. Besides these implants allow young people to lead an active life even after hip replacement.

The surgery takes approximately 1-3 hours, depending on the condition of your hip at the time of surgery

The rehabilitation will begin the next day after the surgery at the hospital itself. Your physical therapist will do a number of exercises to regain your strength and range of motion. You should continue to exercise until your muscles are pain-free and you can walk without a limp. It is a good idea to continue your exercises as a lifetime commitment to keep your muscles strong.

This varies with each person. Most of the people use a walker for about one- two weeks after the surgery and increase their activities slowly. Most people can drive a car, play golf, or go bowling in about ten – twelve weeks.

The materials used for total hip replacement arthroplasty include titanium, cobalt chrome, and polyethylene. To date, there is little evidence suggesting any allergic reaction to these materials.

Yes, you can. But how soon you can do it depends on your comfort level. It is generally uncomfortable to sleep on the side of an operation for a few weeks after the surgery. If you do need to sleep on the operation side, keep pillows between your legs. It shall make the hip stable and shall be more comfortable for you than otherwise.

Active sports should not be resumed for at least 8 weeks after the surgery. You should start slow with short sessions and then slowly increase the activity level as tolerated by your body. Consult your surgeon before you start with active games.

This is very individualistic and impossible to say if you can drive after the surgery and when to begin driving. Although it is safe for your hip if you drive after about 8 weeks. There are few important criteria that must be met for patients to drive safely.

You should be off narcotic pain drugs, be able to get in and out of the driver’s seat comfortably and must have regained the strengths in your muscles. Which side has been operated upon is another deciding factor. Use of the right leg is more in driving and thus people with right hip replaced take longer before they can start driving. You can sit with an expert driver and test if you can comfortably drive before you start all on your own.

This depends upon your comfort level. You can resume sexual activities when your hip is comfortable enough to allow it. You should avoid positions that put too much stress on the hip joints. You can consult a therapist for it.

The life of your new hip joint shall depend upon your physical condition, activity level, and weight. We use FDA approved best implants in the world which have shown to have a life of 35 years in laboratory settings. In real life as well, the implants have shown to last for 30 years or more if taken care properly.

Yes, they usually do. The hospital shall provide you with the letter stating that you have a metal prosthesis fixed inside your hip which shall help you clear through the security checks without facing any problems. You should reach the airport a little early to avoid any inconvenience.
