What is Fibroid Treatment?

Tumors that grow in the muscular wall of the uterus are called "Uterine fibroids". These are benign tumors that are noncancerous in nature and are developed from the smooth muscle tissue of the uterus. This mass can generally develop in childbearing years and may grow quite large and their size may differ from the size of a pea to a lemon. The exact cause of the development of fibroids is still unknown but several factors influence their formation including family history and hormonal changes.

Since fibroids do not produce early symptoms, it is difficult to diagnose them early. It is estimated that every 3 out of 4 women have some or the other form of fibroids in their lifetime. The lady having fibroids may experience symptoms like heavy menstrual flow, prolonged menses, frequent urination, constipation, back or leg pains, difficulty in emptying the bladder, pain in pelvic area etc.

Types of Fibroids:

There are mainly 4 types of fibroids that develop at different locations in and on the uterus.

Intramural fibroids: These are the most common type of fibroids which prominently appear within the muscle lining (endometrium) of the uterus.

Submucosal fibroids: These types of fibroids form in the inner lining (myometrium) of the uterus and may extend into the uterine cavity. These are not as common as other types of fibroids but may cause heavy menstrual bleeding and trouble in conceiving when developed.

Subserosal fibroids: These types of fibroids appear close to the outside of the uterus-partly in the myometrium and partly outside.

Pedunculated fibroids: When submucosal or subserosal fibroids develop a slender base that supports the tumor, they are called pedunculated fibroids.

Uterine fibroids can result in developing two different types of uterine conditions which can be catheterized under as:

A. Cancerous uterine conditions
B. Non Cancerous uterine conditions

Most of the growths in the uterus are typically non-cancerous in origin. Some of the most common non-cancerous conditions are:

  • Fibroids
  • Endometriosis and endometrial hyperplasia
  • Severe menstrual pain and ovarian cysts
  • Uterine prolapse
  • Pelvic inflammatory disease

When is Fibroid Treatment required ?

Treatment of fibroids depends on the age of the patient, her medical condition and obstetric history, size and type of fibroid. The common criteria associated with selection of treatment also depend on the symptoms like:

  • Excess and prolonged menstrual bleeding
  • Pain in the abdomen and lower back
  • Painful intercourse
  • Frequent urination or constipation

Fibroids Treatment Options

Treatment for Fibroids can be classified in two ways:

Medical Management:
GnRH agonists, Progestin, oral contraceptive pills, androgenic agents and anti-estrogens.

Surgical Management:

  • Myomectomy: This procedure is for removal of fibroids but conserves the uterus. There are different techniques of performing myomectomy like Hysteroscopic, laparoscopic and open myomectomy.
  • Uterine Artery Embolization/Uterine Fibroid Embolization (UAE/UFE): The blood flow to the uterus and/or fibroid is blocked, leading to necrosis and a gradual shrinkage of the fibroid.
  • Hysterectomy: It is one of the options which involves complete or partial removal of the uterus and the fibroid. It can be done the conventional way or minimally invasive way.

How is Fibroid Treatment done?

Myomectomy

Laparoscopic myomectomy removes intramural tumors viz., tumors that are deep inside the uterus wall or subserosal which lie outside the uterus. A very small incision gives access to the laparoscope and surgical tools making it a minimally invasive procedure.

Hysteroscopic myomectomy technique removes fibroids that are submucosal with the help of a small camera called a hysteroscope to pass through the cervix into the uterus in order to excise and remove the fibroids.

Open myomectomy is the traditional method with incision large enough to remove the fibroids. This technique is used when other less invasive methods are not an option.

Hysterectomy

Laparoscopic Hysterectomy: The patient is anesthetized. Small incisions of about a centimeter are made on the lower abdomen. The abdomen is then filled with carbon dioxide and a small camera with laparoscope and small instruments are introduced and operated. The gas is later removed and incisions are closed.

Vaginal Hysterectomy: The procedure is performed through the vagina. Both vaginal and Laparoscopic hysterectomy are for situations where the uterus is not too large in size.

Abdominal Hysterectomy: This is the traditional horizontal incisions to remove the uterus and fibroids when it is too large for minimally invasive methods.

Uterine Artery Embolization (UAE): The procedure is done by an interventional radiologist. The patient is sedated and a needle is inserted into the leg and a catheter is connected to the artery and x-ray is taken. Particles of polyvinyl alcohol are injected that block the arterial supply for the fibroid. Thus, the uterine arteries are embolized and the blood supply to the fibroids is cut off, so that the fibroids shrink and disintegrate. The patient has to remain in bed for 6 hours with the leg straight. Pain can be managed with medicines after the procedure.

What is the Recovery after Fibroid Treatment like?

Recovery time for laparoscopic and all minimally invasive procedures require an overnight stay in the hospital. Uterine Artery Embolization procedure involves overnight stay at the hospital but total recovery will happen in 7 to 10 days. Vaginal hysterectomy usually takes 24 hours to stay at the hospital. Mild pain persists for approximately 10 days. Whereas with abdominal hysterectomy, it takes 3 to 4 days of hospital stay and total recovery needs 4- 6 weeks.

What Results Can I Expect from Fibroid Treatment?

85% to 90% of the patients have significant resolution of symptoms such as pain, heavy bleeding etc. Hysterectomy is a permanent treatment where the uterus is taken out and there are no chances of having fibroids after that. The flip side is that the woman cannot get pregnant after hysterectomy. Myomectomy has good results with resolution of all symptoms. Chances of recurrence of fibroids have to be consulted with the doctor.

Fibroids are very common, and the majority of them do not require treatment.

Fibroids are benign tumor growths that are not malignant in the majority of cases. Fibroids are, however, unlikely to become malignant. Also, having fibroids does not mean that there are high chances of uterine cancer, it is a myth.

Approximately 80% of women will have at least one fibroid at some point in their lives, and any woman can get uterine fibroids. Women who have a family history of fibroid are at a higher risk. As a result, African-American women face a larger risk than other women. Obesity or excess weight, as well as diet (women who eat more red meat), are additional risk factors for fibroids development. Fibroids are most commonly diagnosed in women between the ages of 40 and 50. Fibroids are less likely to occur after menopause.

Pregnancy will be more challenging due to the fibroids. Fibroids, on the other hand, do not cause infertility. Even with fibroids, most women can conceive and deliver normally. Fibroids may cause problems during pregnancy or labor. It's possible that you'll need a C-section or that you'll have to deliver your baby early. It is recommended that you see a doctor about your condition.

Hysterectomy is the surgical removal of the uterus and is thought to be the only way to ensure that fibroids do not recur. Fibroids are the most common reason for hysterectomy in women. Despite this, many women are apprehensive about undergoing this operation because it fully eliminates their chances of becoming pregnant. Hormone therapy and birth control tablets may help to alleviate the symptoms. Furthermore, there are a number of modern surgical treatments that solely target the fibroids while leaving the uterus intact. The doctor will offer the appropriate treatment option based on your age, your desire to get pregnant, the size and location of the fibroid and your overall medical condition.

Uterus removal cost in India starts from US $3200 and varies depending on your medical condition, type of surgery technique used, your surgeon, facility and the city where the surgery is done.

The procedure can be consented to by any woman of legal age, but it must be medically warranted. A doctor is unlikely to conduct a hysterectomy on a woman between the ages of 18 and 35 unless it is really necessary for her health and no other treatments are available.
