What is Breast Reduction Surgery ?
The plastic surgery procedure for reducing the size of large breasts and bringing them in proportion to the body, while retaining the functional abilities of natural breasts, is called breast reduction surgery. Scientifically known as Reduction Mammoplasty, the procedure helps in making the breasts lighter and firmer.
The surgery actually works on three levels for the patient – physical, psychological and aesthetic. The procedure entails removal of excess breast fat, glandular tissue, and excision of extra skin, along with lifting and reduction of nipple areola complex, to give more proportionate and shapely breasts.
Breast reduction surgery thus leads to uplifted, perky and much younger looking breasts, minus the excruciating and chronic neck, shoulder and back pain!
Reasons for undergoing Breast Reduction
Breast hypertrophy or macromastia is the major reason for anyone to undergo breast reduction. Some other reasons for women to undergo breast reduction surgery are:
- Breasts that are too large for the body frame leading to poor posture
- Heavy breasts causing back neck and shoulder pain
- Skin infections and irritation under the breast fold
- Heavy breasts make the nipples and areolas point downwards
- Sometimes one breast is larger than the other
- Overtly heavy disproportionate breasts can make a woman conscious and unhappy with her self-image.

Am I a good Candidate?
If you’re physically and mentally healthy, 18 years of age or older, not currently pregnant or nursing, have realistic expectations and are having any of the following concerns about your breasts-
- too large in proportion to your body frame
- heavy pendulous breasts with nipples that point downwards
- one breast that is much larger than the other
- back, neck or shoulder pain caused by the weight of your breasts
you are a good candidate for breast reduction surgery. However, if your problem is only saggy breasts, and you wish to uplift the breast without any reduction in size, Breast lift is the right surgery for you. Please visit Breast lift surgery page to check details.
Breast Reduction Techniques
There are different types of breast reduction surgery techniques and your plastic surgeon would advise the most suitable technique for you, depending on various factors including:
- Your present size and the size you wish to achieve
- Your age
- Presence of any comorbid factors
- Smoking history
- Whether you have completed your family or plan to breastfeed in future
The various techniques of breast reduction surgery include:
Inverted- T or Anchor Breast reduction: It is the standard technique for breast reduction since maximum reduction is possible with this technique. The disadvantage however is that it is quite an invasive method. The incisions run horizontally under the breast crease, then continuing vertically to the nipple and run around the nipple giving the incision an anchor pattern.
Lollipop or Vertical Breast Reduction: This kind of incision results in less scarring and more curvaceous breasts. But it can only be used for moderate breast reductions. It is also called the LeJour breast reduction or short scar reduction. The incision runs around the areola and then continues down vertically to the breast crease.
Liposuction breast reduction: This technique is suitable in females where fat tissue content is more in breast than glandular tissue and skin removal is not required. The skin recovers over time giving a natural look and the sensation of nipple or breast feeding ability is not compromised.
Free nipple graft: The technique is mostly indicated for gigantomastia or severe macromastia. The nipple areola complex usually remains attached to the nerves and blood vessels in most breast reductions, however in case of very large and heavy breasts, the nipple areola complex is completely removed and grafted to a new higher position, with removal of large amount of breast tissue, fat and extra skin.
How is Breast Reduction Surgery done?
Step 1
The first step is general anesthesia for a comfortable and painless surgery.
Step 2
The excess tissue of the breast is removed and reshaped. The areola is repositioned or even reduced if necessary, retaining the blood and nerve supply usually. This gives the breast an uplifted and perkier appearance.
Step 3
The incisions are then closed with suitable sutures and bandaging done.
What is the Recovery after Breast Reduction Surgery like?
It is normal to have moderate to severe pain for a few days after the procedure, which shall be controlled with pain medications. The extent of pain also depends on the magnitude of surgery you have undergone and your individual pain tolerance. Your dressing will be removed in about two to four days and stitches after a week. You can resume non-strenuous daily activities with caution within a week and strenuous activities after 8-12 weeks.
What Results Can I Expect from Breast Reduction Surgery?
Most women are quite pleased after undergoing breast reduction surgery, in fact it is often considered the procedure that results in the quickest and most impactful body image change for women. Many women find that this surgery also relieves chronic back, neck, and shoulder pain. The primary benefit of the operation is functional, but you also get a breast lift, which enhances your appearance and greatly improves your body image and boosts self esteem.

Strenuous exercises and rigorous activities should wait for at least 10-12 weeks even if you feel you have regained the strength and are up to it.
The surgery is done under general anesthesia and you will have moderate to severe pain when you wake up. Your surgeon will prescribe you pain medication to control the pain. It is normal to feel tired after the procedure. Your breasts will probably feel tender, sensitive, swollen and bruised for a few days after the surgery. But you can expect to feel better and stronger each day and should be able to return to routine activities within 10-12 days of the surgery.

How many cup sizes you can go down depends on a lot of factors. This includes how much volume you wish to lose, whether you wish to breastfeed in future, your breast composition, which technique is used for your surgery, your framework etc. You can discuss this with your plastic surgeon to get a specific answer. Generally speaking women can easily lose from one to three cup sizes in a breast reduction.
Yes, breast reduction removes extra fat, skin and tissue from your breasts and aims to bring your breasts to where they should be- making them perkier and youthful.
Breast reduction thus in the process definitely does breast lift also.
The best surgical technique for you depends on various factors. This includes your age, whether you wish to breastfeed or not, how many cup sizes you wish to go down etc. You should have a detailed discussion with the plastic surgeon about the pros and cons of different techniques and then make an informed decision.
You should discuss this with the plastic surgeon before you choose to undergo the procedure. Breast reduction does affect the ability to breastfeed, but the surgeon will modify the technique and spare the milk ducts and glandular tissue as much as possible, if you wish to breastfeed after your surgery. This is a tradeoff- you may not get the best results, but you will be able to breastfeed.
